TenderID

This form is for individuals or caregivers who would like to register to receive a free TenderID card through Clay County Fire & Rescue.

A TenderID card contains a secure QR code that links to important medical information. When scanned by emergency personnel, the QR code can provide quick access to critical details such as medical conditions, medications, allergies, and emergency contacts. The purpose of this program is to enhance emergency response and ensure first responders have access to accurate information when seconds matter.

There is no cost to residents to participate in this program. However, there is a cost to Clay County Fire & Rescue to purchase the cards in bulk. We are prepared to absorb this expense as part of our commitment to community safety, provided the cards are actively used by our residents.

At this time, we are asking individuals who are interested in receiving a TenderID card to submit their contact information using this form. Program updates will be provided via email, and cards will be distributed in the order that registrations are received.
